Of all time, no - they got to a couple of wc semis back in the days of Lato, Zmuda and Boniek.
Of the last 20-30 odd years, probably although Russia have given them a run for their money.
In terms of teams that have qualified for, say, 5 or more tournaments, then it has to be the jocks.

Agree, have never qualified out of their group, ever. This is their 9th attempt. The Poles came 3rd in the World in 1974.
