Little feature on news there about the McCabe case. They showed the lady from the HSE who made the clerical error coming out of the courts. A very attractive lady altogether (albeit heavily pregnant). Iâm giving her the benefit of the doubt

Wasnât mentioned on the news anyway. It said when she realised her error she panicked and went straight to the HSE and the guards to correct the mistake but it was too late the guards had already started the investigation of mccabe. She is a classy looking lady Iâd be surprised if she is up to no good

Allegation of abuse
The tribunal heard that Ms Brophy works with the Rian service in Cavan for adults who have suffered childhood abuse and that she met with Ms D on July 24th, 2013. Ms D told her of an allegation of abuse against Sgt McCabe which had been reported to the Garda in 2007 and did not lead to any prosecution.
Ms D had âeveryday coping difficultiesâ and difficulties with relationships. By the time there was a counselling place available for Ms D, she had moved to the south east.
Meanwhile Ms Brophy had contacted the social worker service in Cavan and asked if theyâd received a report concerning the allegation. She knew Ms D had made a complaint to the Garda.
Upon being told that the social worker service had no record of the allegation against Sgt McCabe, Ms Brophy made a retrospective report.
However when typing in the details, she left details belonging to another client, Ms Y, on the Microsoft Word document. These referred to digital penetration and a threat to Ms Yâs father in the case that Ms Y told anyone of the abuse. It appeared that this report was then forwarded to the Garda.
The witness said that once she had discovered the mistake, she apologised to Ms D, and set about getting the return of the report she had sent to Cavan and also to the service in the south east to which Ms D had been referred. She was later told later that Supt Leo McGinn had said the case had been transferred to a Garda commissioner, outside the region.

Would say it was an honest mistake on her part alright. Tough for her having to be up there in her condition.

Iâm dubious enough but itâs plausible alright. Iâm sure many of TFKâs auditors can identify with using an already completed template to draft a report but accidentally retaining some information relating to the original client. You could easily have submitted a whole report outlining the systems and control environment of a specific insurance company. But, on further investigation and mature reflection, it may have become apparent that you inadvertently detailed the entire process for a competitor business and only changed the company name in the report. A simple mistake to make.

A former manager of mine had a habit of doing up annual appraisals and when you would get it to sign off it would have someone elseâs name on it. Possibly someone who he hadnât worked with in years.
