What happens if the same letter is in a word twice
For example if the word is
EVENT
and I guessed
EARLY
I would get a
Do you just have to assume that the E is still in play for further guesses
Yes and this happens sometimes. There are double letter words, all that.

Anyone consistently getting 3 is cheating. There are surely still absolutely loads of words it could be after only two guesses.
